Health Insurance, Motor Vehicle Claims, and Sliding Scale

Health Insurance:  To learn about your insurance coverage for Acupuncture and whether I can accept your insurance for in network or out of network benefits, email or call me with your full name, insurance company name, member ID#, and your date of birth.

As a courtesy, I also bill health insurance as an out of network provider. 

MVA claims:  I bill for MVA (motor vehicle accident) claims with medical benefits and Acupuncture coverage.  Acupuncture is included in Personal Injury Policies (PIP) in the state of Oregon.  Please bring your insurance information with you to your first visit, including claim number and phone number of the department handling your medical claim. 

**Sliding scale available:  I offer a limited number of sliding scale slots for patients who are experiencing financial hardship.  If this is you, please ask me about the sliding scale.  Donations to support this cause are accepted.  Please inquire for details.
